Provide Relations Liaison at Midwest Palliative & Hospice CareCenter

The Provider Relations Liaison is responsible for establishing relationships, including but not limited to; Hospitals, Physicians, Long Term Care Facilities, Independent Living, Assisted Living and Supportive Living Communities. Within these relationships; the purpose of the Provider Relations role is to educate about Midwest CareCenter services and to increase referrals to Midwest CareCenter, with specific focus on Hospice and Palliative Care."
